Unapproved extension cord. Metal 2*4 utility box was designed to be mounted inside of a wall, and not on the end of an extension cord.

Hazard:
This picture shows a damaged extension cord plug The rubber outer coating has been pulled loose from the plug which results in the possible strain to the end connections. The plug is also broken which possibly exposing live electrical parts once it is energized (plugged in an outlet). This could cause electric shock, even electrocution.
