Description:

Employees are exposed to a fall hazard by accessing a roof of a building using the boom extension of a rough terrain forklift truck and the forks/platform.  A work platform is made by using a pallet attached to forks.

Abatement:

Provide appropriate fall protection for employees working at elevated levels such as a fall protection system and or scaffolds. Provide safe access to work levels such as ladders or aerial lifts. Use equipment for intended purpose according to the manufacturer’s guidelines.
