Abatement:

Employer needs to provide a safe work platform or fall protection system. Reference 1926.502 for Fall protection systems and criteria and practices.

Abatement:

Scaffold poles, legs, posts, frames, and uprights were not placed on base plates and mud sills or other adequate firm foundations.

Abatement:

Employer needs to provide fall protection to employees at all working levels and the fall distance needs to be limited to 6 foot fall distance.  Reference 1926.502 for Fall protection systems and criteria and practices.
Provide cross bracing at all locations where the horizontal members support the outer and inner legs.  See manufacturer's guidelines.
